We are able to perform a number of non-NHS, or private services. These include private sick notes, letter of medical fact, short letter of opinion, short medical form, childminder health form, HGV/Taxi/Pre-employment medicals, GP reports, formal assessment of mental capacity, formal adoption medicals.

Please Note: Full payment is required prior to completion of the report/documents of on booking of appointment for medical examination. The practice does not undertake certification of indentification documents, for example driving licence and passport.
Under their NHS contract, GPs are not required to provide reports or offer an opinion on incapacity for work to anyone else other than Jobcentre Plus of DWP with will write to the GP direct.
